Keystone Rural Health Consortia & Vaccine Requests

Please be patient. If you have already submitted your vaccine request form, you do not need to take any further action. We are doing our best to meet the needs of our patients and the communities we serve based on guidance from the Dept. of Health. Please know that although we would like to accommodate every request for a vaccine that we receive, we are not able to do so at this point. We are diligently working our lists and scheduling vaccines. Please stay safe and continue to follow social distancing and wear your mask! As you can imagine we are being inundated with hundreds of requests, please do not reach out directly to staff, we are unable to take personal requests. Please continue to complete the Vaccine Request Form on our website. http://www.keystoneruralhealth.com

The PA Dept. of Health updated category 1A to include the following:
 Please DO NOT call our offices to schedule a vaccine, please complete the online vaccine request form with the needed information. Please note that completion of this form does not guarantee a vaccine. 

Persons ages 65 and older 
• Persons aged 16-64 with high risk conditions causing increased risk for severe disease: 
 • Cancer 
 • Chronic kidney disease
 • COPD 
 • Down Syndrome
 • Heart conditions, such as heart failure, coronary artery disease, or cardiomyopathies 
 • Immunocompromised state (weakened immune system) from solid organ transplant or from blood or bone marrow transplant, immune deficiencies, HIV, use of corticosteroids, or use of other immune weakening medicines 
• Obesity (body mass index [BMI] of 30 kg/m2 or higher but < 40 kg/m2) 
 • Severe Obesity (BMI ≥ 40 kg/m2) 
 • Pregnancy 
 • Sickle cell disease 
 • Smoking 
 • Type 2 diabetes mellitus
