1. if you have signs or symptoms of a respiratory infection, such as fever, cough, shortness of breath or sore throat, or

2. in the last 14 days, have had contact with someone with a confirmed diagnosis or are under investigation for COVID-19 or

3. if you have traveled internationally within the last 14 days
